A fragmented journey makes the right care harder to find
People’s needs don’t pause between appointments. Yet their care may be spread across therapists, psychiatrists, primary care clinicians, and other providers, each seeing only part of the picture. People can be left navigating the gaps themselves.
The challenge is both continuity and direction: keeping care connected over time, and helping each person reach the provider or treatment that is right for them. Without a shared view of the journey and its outcomes, it is harder to coordinate care or know when a different path might help.
Create continuity around the person
Our first clinical solutions focus on connecting people with their care providers between visits and throughout their treatment journey. The aim is a shared foundation that helps information and follow-up move with the person, rather than stopping at the end of an appointment.
The immediate goal is worthwhile on its own: fewer gaps in follow-up, less information lost between providers, and clearer next steps for people receiving care. Our first clinical solutions are in development around these needs.
Build on those connections to measure what works
Beyond the immediate benefits of continuity, connected care can create a way to follow experiences and outcomes across visits and providers. We want to use those connections to measure outcomes reliably and see where care is working, where it is not, and where needs are going unmet.
That evidence needs to reflect the course of care, not just isolated encounters. It provides the foundation for learning which paths help people and for recognizing effective care.

We aim to use that evidence to power predictive models that help identify which treatments and care paths may work for each person. These models can support clinical decisions and help people find an appropriate next step in their journey.

The same evidence can support payment and incentive structures tied to outcomes. When effective care is recognized and rewarded, providers and health systems have a stronger reason to invest in coordination and the tools that make it possible.

This broader direction builds on four years of research with Vanderbilt and the Wellcome Leap MCPsych program, alongside work with leading health payers on outcome-driven program design. We’re pursuing it step by step, starting with the practical gaps in care people face today.
